Requests for Kentucky Board of Pharmacy address lists should be directed to Kentucky.gov.

The lists available are:

• pharmacists
• pharmacy technicians 
• resident pharmacies
• non-resident pharmacies
• wholesaler / manufacturers
• medicinal gas pharmacies.

Lists will contain name of licensee and address.
