Pharmacist Intern Information
Pursuant to 201 KAR 2:040, Section 2 - An applicant for registration as a pharmacist intern shall register with the Board.

Pharmacist Intern Application [PDF - 45KB] Once you have completed this application, please print, date and sign then submit to the Board office along with a passport sized photo, letter of acceptance into a college of pharmacy [a copy of your FPGEC certificate for foreign graduates] and payment.
Pharmacist Preceptor Affidavit [PDF - 46KB]must be submitted in duplicate within ten (10) days from the beginning of internship. Preceptor Affidavit must be resubmitted in duplicate within ten (10) days if there is a change in preceptor.
Pharmacist Internship Report [PDF - 30KB]The deadline for intern hours completed from September 2010 through September 2011 is October 1, 2011. Hours submitted after the October 1st deadline will not be accepted.

Address/Employment Changes |

All pharmacist interns are required to notify the Board of any changes in address within 30 days of moving. Address changes may be made electronically. Use the ‘Contact Us’ option to submit your address, employment or email changes to the Board. Please provide your license number with your information.
